Colon Examination via Sigmoidoscopy: Procedures, Purposes, and Preparation Expectations

A sigmoidoscopy is a medical test used to examine the lower portion of the colon, known as the sigmoid colon. This procedure is a valuable tool in the early detection of polyps and colorectal cancer.

Preparation for Sigmoidoscopy

Preparation for a sigmoidoscopy typically involves clearing the lower bowel beforehand. This can be achieved through one or two enemas or by following a clear liquid diet for 1 to 3 days, depending on the doctor’s instructions. It's also essential to inform your doctor about all current medications and allergies, as blood thinners like aspirin might require special instructions. In some cases, a level of food restriction may be advised, such as avoiding solid foods.

The Sigmoidoscopy Procedure

On the day of the procedure, you will lie on your left side (Sims’ position) for access and comfort. A digital rectal exam (DRE) is often performed first to assess sphincter tone and check for abnormalities. A sigmoidoscope—a flexible, lighted tube—is gently inserted into the anus and guided through the rectum into the sigmoid colon. Water-based lubricant is used to ease insertion. The scope inflates the colon with air to improve visibility, and a small camera transmits images to a screen. The physician examines the colon lining while slowly advancing and withdrawing the scope.

Post-Procedure Considerations

Individuals may experience mild symptoms after a sigmoidoscopy, such as soreness and mild stomach discomfort a day after the procedure. Sedation is usually not necessary but may be given in some cases to reduce discomfort. You can usually resume normal diet and activities immediately after the procedure.

Additional Procedures

If abnormalities are observed during a sigmoidoscopy, a doctor may order a colonoscopy for a more comprehensive examination. If abnormalities are found, a biopsy may be performed to remove small pieces of tissue for further analysis. Endoscopic mucosal resection (EMR) is a procedure for removing a polyp, but it is not a routine procedure during a sigmoidoscopy.

Risks and Precautions

The sigmoidoscopy is a safe and routine procedure, but people may experience mild side effects during and after the procedure. Potential risks following a sigmoidoscopy include rectal bleeding, perforation of the colon, and severe pain in the abdomen. Anyone experiencing severe symptoms after a sigmoidoscopy, such as severe bleeding or fever, should consult a medical professional.

If a biopsy is performed during a sigmoidoscopy, a person will receive a pathology report with detailed information about the tissue sample and relevant treatment recommendations. Dietary changes for sigmoidoscopy preparation may include a liquid diet for 24 hours before the procedure.

At-Risk Individuals

Certain individuals may be at risk for colon cancers, including those with a family history of polyps or colorectal cancer, inherited syndromes, type 2 diabetes, obesity, physical inactivity, a history of smoking or excessive alcohol use, inflammatory bowel disease, and certain other health conditions.

Performing the Sigmoidoscopy

In most cases, the person performing a sigmoidoscopy will specialize in the digestive system, such as a gastroenterologist, a surgeon, a nurse practitioner, or a physician assistant. The room should be private and comfortable, with all instruments (scope, lubricant, biopsy forceps if needed) ready and sterilized.
